Coverage

• Choose from 1 of the 3 plans in our SmartCare Executive which matches your AXA Group medical plan. Alternatively, talk to your agents, brokers or call us if you are interested in any other individual health products.
• Request for benefits higher than your existing AXA group medical insurance will be subject to underwriting approval, additional terms & conditions may be applied

Eligibility

• The Insured Person must be at or below 60 years old
• Existing AXA group medical insurance member can choose to join the Portable Plan either:
  • within 30 days prior to the termination / leaving the AXA group medical insurance^; or
  • within 30 days prior to any subsequent renewal of the AXA group medical insurance^; or
  • within 30 days of joining the AXA group medical insurance as a new employee^
• Employees may apply to include your family members in this Portable Plan
• Health declaration is required for family members who are not yet covered in the AXA group medical insurance
• All applications are subject to underwriting approval. Exclusions may be applied

 

Remark: 

• ^ Members with less than 12 months of group medical insurance cover will be subject to pre-existing exclusions and waiting periods as applicable to their chosen plan
