Here is a simple step-by-step procedure to follow when you make a claim.

Step 1: Complete a claim form

Complete the appropriate claim form depending on which claim is to be made. All details in respect to member’s HKID card/passport number, policy/membership numbers, name of policyholder, etc. should be duly filled.
 

Step 2: Obtain original documents, medical reports & invoices

Submit all original medical receipts together with invoices/bills, original medical report (issued by the attending registered physician), referral letters and relevant documents.
 

Step 3: Submit required documents along with the claim form

              Once the claim form is completed and all related documents are in place, submit them to AXA
              General Insurance Hong Kong Limited by post to:

              Health Service Team
              AXA General Insurance Hong Kong Limited
              21/F, Manhattan Place, 23 Wang Tai Road
              Kowloon Bay, Kowloon, Hong Kong

 

Making a pre-authorization:

  1. Complete the direct billing/pre-authorization approval claim form.
  2. Submit completed claim form & attach originals of all relevant documents such as medical reports from attending doctor on proposed in-patient treatment/surgery/diagnostic investigations/long term medication to Health Service Team.
  3. Use a new claim form for each separate claim or illness.

 

Making reimbursement for in-patient/out-patient/dental claims:

  1. Complete the reimbursement in-patient/out-patient/dental claim form.
  2. Submit the completed claim form & attach originals of all relevant documents and final detailed hospital/doctor’s bills to Health Service Team.
  3. Use a new claim form for each separate claim or illness.

 

Emergency situation (where pre-authorization is not provided):

  1. Complete the reimbursement in-patient/out-patient claim form.
  2. Submit the completed claim form & attach originals of all relevant documents, final detailed hospital/doctor’s bills and in-patient discharge summary (for admission) to the Health Service Team.
  3. Use a new claim form for each separate claim or illness.

Note: Failure to obtain pre-authorization as recommended/required may prevent us from settling all or part of any claim.
